Annual Screening Program

Dear Parent/Guardian,

The district’s School Health Services program is committed to supporting your student’s academic success by promoting health and wellness in the school setting. One way we provide care is by completing the health screenings mandated by the New York State Education Department.

During this school year, the following screenings will be conducted at school if they have not already been completed by your student’s healthcare provider:

Vision Screenings

  • Distance acuity and near vision:
    Required for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7, and 11.
  • Color perception:
    Required for all newly entering students.

Hearing Screenings

  • Required for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7, and 11.

Scoliosis Screenings

  • Required for girls in Grades 5 and 7
  • Required for boys in Grade 9

Health Appraisals (Physical Examinations)

A current physical examination, including Body Mass Index and Weight Status Category information, is required for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7, 9, and 11. We encourage families to provide this documentation annually.

Dental Certificates

A dental certificate is requested for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7, 9, and 11.

If a physical examination is not submitted to the school nurse, your student will be scheduled to see the school physician during the school day. This examination is not intended to replace regular medical care provided by your child’s primary healthcare provider.

If any school-based screening identifies a concern or the need for medical follow-up, written notification will be sent home. Please contact your school’s Health Office with any questions or if you require additional information.
