•  Registration Requirements

    All new entrants to the school district must register at the Family Information Center located at 500 North Street on the campus of the White Plains High School. Parents/guardians should call the Center at (914) 422-2038 (Monday - Friday, 8:00 a.m. to 4:00 p.m.) to make an appointment to register.

    Note: A student cannot be assigned to a school if any documentation is missing.

    The following is required for all registrations:

    • Proof of residency in White Plains (see information below);
    • Official verification of the date of birth (birth or baptismal certificate or passport);

    Proof of guardianship/Custody, if applicable;

    School Records (transcript, including report card, test results);

    •  Immunization Record (see information below) which must include:

    1. physician's signature and/or the stamp of the health clinic;
                       - and -
    2. month, day and year for each shot.

     
    To Verify Residency at the Time of Registration

    NOTE: Education Law (Section 3202.1) states that the residency of the parent is the official residency of the student.

    Parents/guardians are responsible for payment of tuition if the parents' residency is not within the White Plains City School District. If children move out of the district, their parents/guardians are responsible for withdrawing them in accordance with district policy or for paying tuition.

    HOMEOWNERS:

    Two documents required:

    1. signed closing statement or a deed   -and-
    2. a recent utility bill (Con Edison) or a recent telephone/cable bill (or notification of hook-up order)

    RENTERS:

    Two documents required:

    1. lease signed by both parties
      -and-
    2. a recent utility bill (Con Edison) or a recent telephone/cable bill (or notification of hook-up order)

    If the lease is not available, a notarized Landlord Affidavit for Renters (call the Family Information Center at (914) 422-2038 for more information about this) must be completed and returned with a rent receipt from the landlord, plus a recent utility bill or telephone/cable bill.

    When Parent(s) and Student(s) Live with a Friend or Family Member:

    Three documents required:
    1. notarized Residency Affidavit (call the Family Information Center at (914) 422-2038 for more information about this)
      and
    2. lease signed by both parties or signed closing statement or deed verifying the residency of the friend or family member
      and
    3. recent utility bill (Con Edison) or recent telephone/cable bill of the friend or family member
    Proof of Immunization to be presented during Registration:
    The New York State Education Department mandates that school districts must enforce Public Health Law requirements for immunization of children attending school.  Proof of immunization for the diseases listed below must be presented during the registration:

    A Tuberculin Screening Form must be completed and signed by the child’s physician.  If a tuberculosis test is given and the result is positive, written documentation of follow up by a physician is required.  The test should be administered within a year of registration.

    • Physical Exam record is required and should be done within 12 months of school entrance
    • Dental Record indicating a comprehensive dental examination is requested 

    Note: A certificate of medical or religious exemption may be accepted.
