•  Daily Health Screening for Staff and Students

    The safety of the staff, students and their families remain the White Plains City School District’s top priority.  

    To prevent the spread of COVID-19 and reduce the potential risk of exposure, we require your participation in a daily health screening.  

    Your participation is important and will help us take precautionary measures to protect our school community.  

    We ask that you complete this screening every day on behalf of yourself or your child(ren) prior to entering one of our school buildings.  

    All district staff and parents/guardians will receive a link to the district’s daily health screening form via text or e-mail by 5 a.m. every weekday. A reminder will be sent to those who have not yet responded by 6 a.m. You must fill out this form every morning.

    Based on your response, you will be informed if you should report to work or school and enter our facilities.  

    If any individual presents with a temperature of 100.0 or greater, the individual will be denied entry into the facility or sent directly to a dedicated area prior to being picked up or sent home.  

    Frequently Asked Questions 

    Q: What if a staff member answers yes to any of the symptoms? 
    A: They will immediately be prompted to stay home, contact their health care provider and the school coordinator. Your information will be routed to the health office. A nurse or trained health aide will call you to discuss next steps. You may be encouraged to contact the department of health. The school building will make arrangements for substitute teachers, if needed.  

    Q: What happens if a parent/guardian answers “yes” to any of the symptoms on behalf of their child? A: They will immediately be prompted to stay home, contact their health care provider and the school nurse. A nurse or trained health aide will call you to discuss next steps. You may be encouraged to contact the department of health or given instructions for remote learning.  

    Q: What happens if an employee or parent/guardian fails to respond to the heath check questionnaire?  
    A: The district has staff members who will be responsible for following up with individuals who have not responded. Should an individual fail to respond and come to our grounds, they will be immediately asked to take their temperature and complete the health questionnaire.  

    Q: Do I have to take my temperature (or my child’s temperature) every day?  
    A: Yes. Every attempt will be made to encourage parents to take temperatures on their children prior to the arrival at school. If a parent/guardian is not able to or forgot, they need to contact the school to ask the health office to take the child’s temperature for them.   

    Q: What if I cannot fill out this form electronically?  
    A: Please contact the main office of your school building. The district will provide you with paper questionnaires on a weekly basis.  

    Q: What is the Statement of Assurance?  
    A: We ask that parents and guardians complete a Statement of Assurance prior to start of school by Friday, Sept. 11. This statement acknowledges that you will conduct a daily health screening with your child(ren) everyday they come to school.
